City of Toledo Launches After-Action Review Project

In May 2025, the Toledo City Council gave final approval for the launch of an After-Action Review (AAR). The AAR Project will focus on actions surrounding the leadership changes that occurred between September 2024 and March 2025.  It will:

  • Assess the organizational context, culture, systems, and processes related to the multiple personnel actions over the past year involving city leadership.
  • Provide observations and recommendations for improvement to help set a foundation for success for City Council and the new city manager.

What is an After-Action Review?

An after-action review is typically part of an emergency management response to a real-world incident and is a data-driven process to guide organizational learning.  It is designed to rapidly gather information from key stakeholders to learn from what happened (both successes and failures), assess why things happened the way they did, and help an organization improve its performance moving forward. 

The AAR will not focus on individuals or individual performance.  Instead, it will assess organizational performance and examine ways in which the City of Toledo’s culture, systems, and processes can evolve and improve.

What will happen as part of the After-Action Review?

The AAR Project will launch this month (July) and is expected to be completed by this fall.  The City has hired Jensen Strategies, an independent consulting firm based here in Oregon, to conduct the AAR.  Project activities include the following:

  • Key Stakeholder Interviews
  • Staff Workshop
  • Draft Report
  • Council Presentation
